JOB OVERVIEW:
The Assistant Care Team Manager– LPN is responsible for providing support and assistance within the scope of practice in the coordination of duties and supervision of nursing staff as well as providing professional nursing care to patients. Responsible for the overall day to day staff assignments and management of patient flow to ensure excellent customer service.

ESSENTIAL FUNCTIONS:

  1. Participates in the interview, hire, and training and orientation processes of the nursing staff
  2. Participates in the performance evaluation process with the Care Team Manager
  3. Ensures nursing practice of the staff and own practice is in accordance with nursing standards, local, state and federal guidelines and organizational policy
  4. Ensures compliance with all infection control, safety and confidentiality guidelines (OSHA, DOH, HIPAA, etc.)
  5. Assigns duties as needed including daily break schedules for team
  6. Investigates variances and patient and/or family complaints
  7. Develops and performs corrective actions to resolve issues or refers more complex issues to Manager
  8. Supports quality improvement activities, including data collection and committee participation
  9. Supports implementation of programs and procedures developed at an organizational level
  10. Demonstrates responsibility for his/her own professional development and participates in medical group training programs, CME/CEU conferences and in-service programs as appropriate
  11. Obtains, verifies and records health history of patients as needed to support patient flow
  12. Convenes daily huddles and supervisors the Medical Assistants to ensure implementation of physician directed preparation of patients for scheduled visits
  13. Ensures staff maintains excellent customer service when caring for patients
  14. Ensures clerical staff checks patients’ insurance eligibility 3 days prior to scheduled visits and on day of visit for visits scheduled within 2 days of appointment
  15. Monitors inventory of medical supplies and orders when necessary
  16. Reports malfunctioning machinery to Administration
  17. Provides feedback to staff on a regular basis
  18. Disseminates and enforces departmental and office/group procedures in conjunction with management
  19. Interfaces with physicians, management and staff members
  20. Promotes wellness by providing patient education materials and communicating physician advice and/or instructions
  21. Performs all related nursing duties as assigned by the physician
  22. Provides basic patient care by administering medications
  23. Assists other healthcare professionals in performing exams
  24. Follows established procedures for infection control
  25. Ensures staff coverage is consistent with patient volume
  26. Promotes and facilitates the team approach to patient care through appropriate, timely and professional communication and by leading by example
  27. Organizes patient flow and apprises patients in waiting room when delays occur
  28. Performs all other related duties as assigned by supervisor

SKILLS REQUIRED:

  1. Excellent client service and public relations skills.
  2. Knowledge of computer systems and applications.
  3. Knowledge of organizational policies and procedures.
  4. Communicates clearly and effectively with patients, medical staff, staff members and the public.
  5. Ability to read, interpret and apply policies and procedures.

EXPERINCE REQUIREMENTS:

  1. Minimum of two (2) years administrative experience.
  2. Experience in Medical Office preferable

EDUCATION REQUIREMENTS:

  1. Completion of NYS approved Practical Nursing program.
  2. Current New York State Licensed Practical Nurse License.
